Otis Dewey Mullenax O.D. Otis Dewey Mullenax, age 82, passed away on June 5, 2013 Born June 15, 1930 in Scott Arkansas. Retired from Salt Lake City Street Department. He loved to camp and fish. Survived by his wife, LaVaughn, children: Lolan & Janet, Larry , Carla & Michael, Elayne & Robert; grandchildren: LaDawn, Kyle, Andrew, Cody, Kim, Tyler, Kristen, Amanda, Bobby; great-grandchildren, Kayleigh, Gage and Sadie. Special thanks to the VA Hospital, doctors, nurses and staff for all their great care. There will be a celebration of his life on Saturday, June 8, 2013, 11:00 a.m. - 3:00 p.m. at Arbor Manor, 2888 West 4700 South (behind Hires). Please no flowers.
